Application Process

We welcome applications from candidates who will have successfully completed training in an ACGME-accredited residency program in Internal Medicine or Family Medicine by July 1 2020, are or will be board-certified or board-eligible at that time and currently have or are able to obtain a license to practice medicine in Ohio.

Applications are accepted through the ERAS program, or submit the required information to [email protected].

If you are interested in joining our Hospice and Palliative Medicine program as a fellow for the 2020-2021 academic year, please provide the following:

  • Curriculum Vitae (must be month/year specific)
  • A copy of your current state medical license (if applicable)
  • A copy of your USMLE, Comlex and/or National Board Certificate
  • A copy of your Medical Diploma
  • Certified medical school transcript
  • Copy of all board certifications
  • Completed Universal Application
  • A personal statement outlining your reason for pursuing a career in palliative medicine, your experiences with palliative care, and your career goal.
  • Three letters of reference (one from your current or prior program director). These letters must be dated after July 1, 2019, and attest to personal qualifications, scholastic achievements, and clinical capabilities as related to palliative care.
