Hospice & Palliative Care Medicine Fellowship

Comprehensive training in caring for patients with life‑limiting illnesses.

The fellowship is based in the Department of Medicine at MetroHealth Medical Center and Case Western Reserve University.

The MetroHealth system includes an academic medical center affiliated with Case Western Reserve University and provides tertiary through primary care to patients from a wide range of socioeconomic backgrounds. Our fellowship in hospice and palliative medicine is conducted in collaboration with Reserve Care (formerly known as Hospice of the Western Reserve), one of the largest and oldest hospices in Ohio.

The Hospice and Palliative Medicine Fellowship at MetroHealth is supported by experienced faculty with extensive backgrounds in both teaching and the delivery of hospice and palliative care. Fellows gain broad clinical exposure across inpatient and outpatient palliative care settings, as well as hospice care.

Inpatient consultation services include patients on medical-surgical units and in a variety of critical care environments, including medical, cardiac, surgical, trauma/neuro, and burn units, within a Level I trauma center.

A key component of the fellowship is the partnership with Reserve Care (formerly Hospice of the Western Reserve), one of the largest nonprofit hospice organizations in the country. Through this collaboration, fellows train at state-of-the-art inpatient hospice units and work closely with interdisciplinary hospice teams in both inpatient and home-based hospice settings. Fellows also gain experience working with the pediatric hospice team, expanding their exposure to specialized patient populations.

Reserve Care has a long-standing history of leadership in hospice care in Ohio and, through its Hospice and Research Institute, has provided education and programming to healthcare professionals and the community for over two decades. The organization delivers hospice services to both adult and pediatric patients through home-based care as well as residential and inpatient hospice facilities.

These combined training environments provide a rich and diverse clinical experience, supported by strong educational resources and interdisciplinary collaboration. Fellows spend dedicated time at both MetroHealth and Reserve Care, allowing for a well-rounded and immersive training experience.

Faculty across both institutions are committed to teaching, advancing high-quality palliative and hospice care, and supporting fellows in developing the skills needed to provide compassionate, patient- and family-centered care.

Program Aims

  • Provide high-quality training grounded in best-practice models, with an emphasis on developing fellows’ skills in self-assessment, reflective practice, and lifelong learning.
  • Prepare fellows to deliver exceptional, compassionate care and to become leaders in hospice and palliative medicine, while promoting emotional resilience, well-being, and professional fulfillment.
  • Address the needs of underserved populations by advancing equitable access to palliative care and fostering an understanding of the role of hospice and palliative medicine in improving population health through quality improvement initiatives.
  • Promote effective interdisciplinary collaboration by integrating care across MetroHealth Medical Center, Reserve Care (formerly Hospice of the Western Reserve), and affiliated regional partners, ensuring a coordinated and patient-centered approach to care delivery.
