Geriatric Medicine Fellowship

MetroHealth's one-year Fellowship in Geriatric Medicine educates internists and Family Practitioners to lay the foundation for future practitioners to apply their skills.

MetroHealth’s one-year Fellowship in Geriatric Medicine is designed to educate internists and Family Practitioners in the discipline of geriatrics. The expertise attained through this training program will lay the foundation for future practitioners to apply their skills across the continuum of care. Our physicians will be competent clinicians, academicians, clinical researchers, and administrators able to provide care in a variety of clinical settings, including acute, chronic, inpatient, and outpatient, as well long-term care. 

The program provides a myriad of clinical settings in the best available training sites, allowing participants to learn and practice geriatric medicine. This environment fosters learning and teaching, and helps prepare students for future academic faculty positions in geriatric medicine. The successful completion of this ACGME-approved program qualifies fellows to sit for the CAQ (Certificate of Added Qualifications in geriatric medicine) given by one's primary discipline, ABIM or ABFP. Geriatric Services within the MetroHealth System

Nursing Homes

MetroHealth's skilled nursing patients are treated at a new multimillion-dollar skilled nursing and rehabilitation center in collaboration with ProMedica on the Old Brooklyn Campus. This state-of-the-art facility has 96 beds.

This facility provides medically complex and intensive rehabilitation services for short-term patients transitioning from hospital to home, in addition to 24-hour skilled nursing care. Old Brooklyn will become a hub for healthy aging resources that not only includes skilled nursing and rehabilitation services provided by ProMedica, but also offers convenient access to inpatient rehabilitation services, geriatricians and geriatric specialists.

Senior Health and Wellness Clinic

The following is a list of health care services available through Senior Health:

  • Comprehensive medical care
  • Dementia evaluation
  • Falls assessment and consultation, including physical and mental health and psychosocial well-being
  • Geriatric primary care
  • Incontinence evaluation and treatment
  • Medication evaluation
  • Social work counseling
  • VNA hospice

PACE

PACE is a managed health care program that integrates acute and long-term services for frail older persons who have multiple health problems and require a structured, supportive set of services to remain in the community and avoid frequent hospitalizations and nursing home placement. A multidisciplinary team provides both preventive and ongoing care at the Day Health Center, in participants' homes, and in a hospital or nursing home when necessary.

Aims

  • Educate future geriatricians in the discipline of geriatrics to serve as educators, clinicians, leaders and administrators who are able to disseminate the knowledge of Geriatrics to the medical community through their practices, involvement and academic acumen
  • Establish a program of excellence in senior care that engages the community and its resources to provide comprehensive assessment and competent primary care, consultative, transitional and home care
  • Enhance the care of seniors by providing comprehensive multidisciplinary care across the continuum of care that empowers them to live their life at the highest level of function and independence
  • Promote an environment of wellness for faculty and fellows through lifelong learning, collegiality and camaraderie
  • Serve seniors of all demographics, levels of wellness or frailty regardless of financial status or location of residence and address holistically the individual multifaceted needs of the geriatric patient based on their particular goals of care
