Dr. Horwath has been Professor and Chairman of the Department of Psychiatry, Case Western Reserve University at the MetroHealth System since 2010. In this position, he has expanded the psychiatric service, started an MAT Clinic and other substance abuse treatment programs. He developed the mental health service at the Cuyahoga County Jail and, with the support of the Woodruff & PEG Foundations, the ADAMHS Board and Department of Justice, formed an integrated reentry program that provides post-release mental health, primary care and substance abuse treatment to incarcerated persons with severe mental illness. Dr. Horwath has published more than 50 peer reviewed articles and book chapters.

Ewald Horwath and Charles Emmerman Dr. Horwath was selected as the Mental Health Professional of the Year by the Alcohol, Drug Addiction and Mental Health Services (ADAMHS) Board of Cuyahoga County in 2012. He was named the Mental Health Innovator of the Year by the National Association on Mental Illness (NAMI) – Greater Cleveland in 2016. He received the Impact Award for Excellence in Mental Health from the Margaret Clark Morgan Foundation in 2017.
